At Shoreline Family Health Care, we offer patients one convenient location close to home that provides expert medical and behavioral health services for the whole family, provided by a team dedicated to your overall physical and mental health. Licensed practitioners will provide primary and urgent care, outpatient counseling, care coordination, laboratory testing, and an on-site pharmacy. Our personalized, integrated model of care places each patient at the center of a team of caregivers with access to a wide range of services. Teams are made up of medical providers, care managers, mental health providers, and other clinical specialists. Patients are encouraged to share in decision-making and to be actively engaged in their treatment plans. Shoreline Family Health Care believes that everyone should have access to high-quality medical and behavioral health care, regardless of ability to pay.
