Rafael Fernandez Bohorquez, MD, graduated from the Universidad del Zulia in Maracaibo, Venezuela, earning his medical degree in 2004. He soon relocated to Illinois, completing his residency at the Chicago Medical School at Rosalind Franklin University in 2009. Dr. Fernandez Bohorquez joined Kelsey-Seybold as a board-certified internist in July 2025. As a primary care physician for adults, Dr. Fernandez Bohorquez provides preventive care and treats most chronic diseases - like diabetes, hypertension, mental health concerns, chronic pain, and others, encouraging lifestyle changes with the use of medications when appropriate. Dr. Fernandez Bohorquez is also an addiction medicine specialist who can assist patients with any type of substance use or addiction.
