The goal-directed health care model proposes an approach that uses patients’ life and health goals to guide health care professionals in cocreating personalized care plans that are responsive to the shared priorities, needs, preferences, and values of patients.
So, in other words, Goal-Directed Primary care is figuring out a plan that best works for the individual. And if that individual requires a naturopathic/holistic or allopathic approach, we tailor your health goals to those treatments and needs.
