Valley Oaks Hospice

Valley Oaks Hospice strives to improve the quality of life for patients in the final stages of life-limiting illnesses with compassionate in-home care in Walnut. Led by the medical director, a team of doctors, nurses, aides, social workers, chaplains and rabbis, and volunteers addresses each patient’s individual needs, providing physical comfort through pain management and physical therapy, emotional support, spiritual services, and much more. At Valley Oaks Hospice, the team knows how important family and loved ones are to a patient’s care—that’s why they are included in the care plan, with assistance such as crisis counseling and bereavement services available to help them through these times. Care advisors are available to patients and their families around-the-clock, even on weekends and holidays, for added peace of mind.
