Manon Desjardins, BSN, RN, CNP, is an advanced practice registered nurse at University Hospitals Harrington Heart & Vascular Institute. As part of the institute’s Advanced Heart Failure & Transplant Center team, Manon manages the ongoing care of patients with heart failure, which includes assessing patient symptoms, optimizing medications, providing education on self-management strategies, monitoring vital signs, coordinating care with other healthcare providers, and working to prevent hospital readmissions by actively addressing potential complications and promoting lifestyle modifications.
